BIA-ALCL and Textured Implants: The Facts for UK Patients

What it is

Breast Implant-Associated Anaplastic Large Cell Lymphoma — a rare cancer of the immune system (not breast cancer) arising in the capsule around, overwhelmingly, textured-surface implants. Typical presentation: late-onset swelling from fluid around the implant, usually years after placement. Caught at that stage, treatment — capsule and implant removal — is highly effective.

What happened with Allergan

In 2019, Allergan's Biocell macro-textured implants — the surface most associated with BIA-ALCL — were recalled worldwide and withdrawn from the market. The UK's MHRA tracked cases and issued guidance in step with international regulators.

The actual risk, in numbers

Estimates for macro-textured devices run in the region of 1 in 2,000–20,000 depending on surface and dataset — rare, and concentrated in specific implant types. Crucially for UK guidance: routine removal of intact textured implants in symptom-free women was not recommended; the risk of unnecessary surgery was judged higher than the risk being avoided.

So what should you actually do?

• Find out what you have. Implant card, clinic records, or imaging-plus-history if paperwork is gone. "Textured or smooth" is question one.
• Symptom-free with textured implants: awareness, not alarm — know the late-swelling sign, keep routine checks.
• New swelling, fluid or a lump — any implant, any age: prompt assessment. In the UK, that's a GP referral; the finding is investigated seriously and quickly.
